Waiver & Release

I voluntarily choose to participate in the 5th DMVYA Cup. I understand that soccer is a physical sport involving risks of injury, including but not limited to sprains, fractures, concussion, permanent disability, or, in rare cases, death. By signing this waiver, I acknowledge and agree that:

1. 1I am physically fit and medically able to participate in this tournament.
2. 2I assume all risks associated with participating in the event, including injuries, accidents, illness, weather conditions, and any other unforeseen circumstances.
3. 3I release and hold harmless DMV Youth Academy (DMVYA), tournament organizers, volunteers, referees, sponsors, venue owners, and affiliates from any claims, liabilities, damages, or expenses arising from my participation.
4. 4I understand that the tournament organizers are not responsible for lost, stolen, or damaged personal property.
5. 5I authorize tournament staff to obtain emergency medical treatment for me if necessary. I understand that I am responsible for any medical expenses incurred.
6. 6I grant permission to DMVYA to use photographs, videos, or other media of me taken during the tournament for promotional, marketing, and social media purposes without compensation.
7. 7I agree to follow all tournament rules, referee decisions, and the Code of Conduct. Failure to do so may result in suspension or removal from the tournament without refund.

I have read this waiver carefully, fully understand its contents, and sign it voluntarily.
